When submitting an online application, please be prepared to provide the following documents electronically.

• Recent photograph of the applicant (ideally a headshot where we can see their face clearly);
• Photocopy of applicant's birth certificate or passport, and, if not born in Canada, proof of permanent residence or study visa;
• Photocopy of parent's proof of Canadian citizenship status or permanent residence status or study/work visa;
• Proof of residence provided with a copy of a parent's driver's license or Hydro/Gas bill with current address;
• Student Reference Form from current core subject teacher (template will be provided in your application checklist); 
• School report cards/progress reports: Previous final report card and most recent school report card/progress report, if applicable;
• Any Individual Educational Plans (IEP) assigned to the applicant (if applicable);
• Any educational related assessments of the student, including but not limited to psychologists, counsellors, etc. (if applicable);
• Any other pertinent information regarding the applicant's educational needs;
• Custody order (if applicable);
• Court-appointed guardianship (if applicable).
